Heart-type fatty acid-binding protein (H-FABP), a cytoplasmic protein abundant in cardiomyocytes, is highly cardiac-specific, but also expressed at low concentrations in tissues other than the heart. Following myocardial ischemic injury, H-FABP can be detected in the blood as early as 1-3 hours after the onset of chest pain, peaking at 6-8 hours and plasma levels returning to normal within 24-30 hours. Cardiac fatty acid-binding protein consists of 132 amino acids with a molecular weight of 15 kDa. H-FABP can be used as a good biomarker for myocardial infarction detection with high sensitivity and detection rate. Especially within the first 6 hours of an acute ischemic event, it facilitates rapid risk stratification and earlier prediction of patient outcomes.
